Project Details

What We Need
  • Original design for one flyer provided in a high-resolution format (jpeg, pdf, etc.)
  • Accurate, print-ready specifications
  • Note: This project does not include printing
Additional Details

The Western Connecticut Area Agency on Aging offers free, six-week Chronic Disease Self-Management Education (CDSME) workshops for adults with diabetes, chronic pain and other chronic conditions. We have flyers that we've been using for years for our Live Well programs, but would like to update them. A bonus would be if they could be tweaked for all five AAA's in CT with our individual logos or other partners' logos and names on them. Thank you!

What We Have In Place
  • We currently have sample flyers we've already been using, which should make it easy for you to get started. We also have a good color printer to make our own copies, and the ability to provide any other information you need.
How This Will Help
This project will save us $1,792 , allowing us to have more funding for volunteer leader stipends and $25 grocery gift cards for those who complete the six week Live Well with Diabetes workshop in either English or Spanish.

We are trying to get the word out about these free self-management health programs and recruit a diverse group of mostly older adults to take our workshops. We are especially targeting lower income, inner city minority seniors in Waterbury, CT, but some of the towns we serve have a different demographic. Having different or interchangeable photos to put on the flyer would promote inclusivity and help with participant recruitment.
